The health services provided in the Family Health Xtras^TM program are NOT insurance and are not intended to replace insurance. They are reduced Fee For Service programs that provide discounts at certain providers. Members must pay the entire reduced cost for the health services performed at the time service is rendered unless other arrangements are made with the provider's office. Family Health Xtras^TM does not pay providers.

This program is not protected by any Life and Health Guarantee Association. Family Health Xtras^TM is not considered a program for purposes of creditable coverage. A certificate of creditable coverage will NOT be issued upon termination of a discount program.

Family Health Xtras^TM is NOT available to residents of Florida, Oklahoma, Kansas, Indiana, South Carolina, Connecticut, Montana and Nevada or where prohibited by law.
